What documents should I keep permanently?

Keep permanently: filed Articles of Organization (downloadable from Sunbiz.org), all amendments, operating agreement and all amendments, EIN confirmation, and annual reports. Keep for 7+ years: tax returns, financial records, and correspondence with the Division of Corporations.

Is the "Certificate of Formation" the same as "Articles of Organization"?

Other states use different terminology. Florida calls its LLC formation document "Articles of Organization." Texas, Delaware, and some others use "Certificate of Formation." They serve the same purpose — creating the LLC under state law. If a bank or vendor asks for your "Certificate of Formation," provide your filed Articles of Organization from Sunbiz.org.
